Donations FAQs:

 How do I make a donation? Make a secure online donation 

1) You can call the Development Office: (631) 465-6350 and pay by credit card or request a donation envelope to be sent to you, OR

2) Please make checks payable to the Good Shepherd Hospice and mail to:

*Please include the name of the person to be memorialized and the name and address of the person you’d like notified.

What if I don’t know the families address for notification?

If the person to be memorialized was on our hospice program we will send a card to the caregiver we have on file. If they were not our patient, we can send the card to you to deliver.

I made a mistake online, how do I correct it?

Call the Development Office at (631) 465-6350.

What if this is a donation from a group?

That is not a problem. Please include all names and the family will be notified with each person’s name.

Where does my donation go?

It goes directly back into patient care. Specific programs you may support are listed in our online donation page.

What if the person was not on your hospice program?

Please include the name of the person to be memorialized and the name and address of the person you’d like notified and we’ll take care of it for you!

Deferred & Planned Giving - Leave A Legacy
