Good Shepherd Hospice focuses on easing the physical, emotional and spiritual suffering in the final stage of a terminal illness. Our interdisciplinary team provides expert, compassionate care to support patients and families through the final phase of life.

Hospice can benefit anyone who has a physician-certified terminal illness—defined as having a life expectancy of six months or less with no plan to receive further treatment. Our hospice specialists care for patients of all ages with all types of conditions and diseases, including Alzheimer's disease, cancer, cardiovascular disease, pulmonary disease and neuromuscular diseases. Our programs are tailored to meet your specific needs and medical condition.

We come to you—wherever you live. Most patients receive hospice care in their own home or the home of a loved one. But we also provide our services to assisted living facilities, skilled nursing facilities, hospitals and our centers dedicated to inpatient hospice care.

When a patient’s medical needs are too complex to be managed at home, we provide short-term inpatient care at our Hospice Centers in Port Jefferson (on the campus of St. Charles Hospital) and in Rockville Centre (within Mercy Hospital). These specialized centers provide intensive medical management by hospice professionals in a home-like environment that maintains focus on the comfort of the patient and family. The patient returns home to continue hospice care once symptoms are under control.
