- Patient and family desire

of six months or less if the disease follows
its normal course
- Patient is not receiving
curative therapies
- Patient has a primary
physician who will continue to provide care,
and will certify the patient’s prognosis

is suitable for meeting the patient’s
needs.

- The focus is on palliation
rather than cure
- The patient need not
be homebound

are the focus of care
- Medications related
to the terminal illness for pain and symptom
control, provided at no cost and often delivered
to the home
- The plan of care is
developed and implemented by an Interdisciplinary
Team
- In home continuous care
nursing is provided in time of crisis
- Bereavement support
for 13 months
- 24 hr on-call
RNs for telephone support or emergency visits
