• Patient and family desire hospice care
• Patient has a life expectancy of six months or less if the disease follows its normal course
• Patient is not receiving curative therapies
• Patient has a primary physician who will continue to provide care, and will certify the patient’s prognosis
• Patient has a primary caregiver
• The home environment is suitable for meeting the patient’s needs.

• The focus is on palliation rather than cure
• The patient need not be homebound
• The patient and family are the focus of care
• Medications related to the terminal illness for pain and symptom control, provided at no cost and often delivered to the home
• The plan of care is developed and implemented by an Interdisciplinary Team
• In home continuous care nursing is provided in time of crisis
• Bereavement support for 13 months
• 24 hr on-call RNs for telephone support or emergency visits

• What is the prognosis and co-morbidities?
• What is the understanding of the patient and family regarding the diagnosis and prognosis?
