POSITION SUMMARY:
Provides personal care, under the supervision of the
appropriate professional staff that assists the patient/caregiver
in the achievement of optimal physical and emotional
comfort.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Provides personal care services to an assigned
patient caseload in accordance with the plan of care
established by hospice nurse and interdisciplinary
care team.
- Performs simple health procedures taught and monitored
by a hospice nurse, i.e., reinforce clean dressing,
use of bed pan, check urinary catheter, change colostomy
bags, take vital signs, etc.
- Applies safety principles and proper body mechanics
to the performance of specific techniques of personal
and supportive care.
- Observes, reports and documents changes in the patient’s
status promptly to the Hospice Nurse or Team Manager.
- Assists with prescribed exercises, which have been
taught by appropriate professional personnel.
- May assist patient in taking self-administered medication
prescribed by a physician.
- May make beds and perform other tasks to provide
suitable environment for patients.
- Prepares and provides adequate nutrition and fluid
intake according to the plan of care. Feeds the patient
when necessary.
- Performs homemaking services (clean, dust, wash
dishes and light housekeeping tasks) that facilitate
a clean and safe environment for the patient.
- Demonstrates good utilization of basic infection
control practices and follows Hospice policy and procedures.
- Provides temporary respite for family/primary caregiver
during assigned work time.
- May do errands for the patient, such as shopping
for food.
- Keeps an up to date record of activities and the
patient’s condition to be submitted to the hospice
nurse and incorporated into the patient’s record.
- May accompany the patient to and from the doctor’s
office, however, someone else, approved by the hospice,
must drive other than the Home Health Aide.
- Attends required inservice programs.
- Employee conducts himself/herself in conformity
with the HIPAA Compliance Program and applicable institutional
policies and procedures for patient privacy. In addition,
employee conducts himself/herself in conformity with
the CHS Security policies and procedures.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Satisfactory completion of a Home Health Aide Training
Program approved by the New York State Health Department.
- Certification as a Home Health Aide in New York
State.
- Basic understanding and commitment to the hospice
concept of care for the terminally ill.
- Ability to deal with own and patient/family stress.
- Ability to maintain a high level of confidentiality
concerning patient and family information.
- Willingness to travel to the homes of patient/family
and to provide own transportation. Must possess a
current, valid New York State driver’s license.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Must be able to read 12 point font or larger type.
- Must be able to hear and speak in a manner that
is understood by most persons.
- Must be able to sit, stand, stoop, walk, bend and
kneel.
- Must be able to lift, push or pull 25 to 50 lbs.
- Must be able to transfer patients who weigh 150
to 175 lbs.
